Abstract

Due to the coronavirus disease 2019 (COVID-19) pandemic, people's physical activity has declined by 35% because their time spent at home has increased. Decreased physical activity can increase both the number of people with type 2 diabetes annually by more than 11.1 million and the number of deaths by more than 1.7 million. Thus, exercise is important during the COVID-19 pandemic, especially in relation to diabetes. If a person with the COVID-19 virus lacks exercise, the risk of hospitalization is increased by 2.2 times and the risk of death by 2.5 times. In the context of COVID-19, the purpose of home training for patients with diabetes mellitus is to reduce body fat percentage, increase muscle function and mass, and improve insulin resistance. The recommended home training program is a circuit-based exercise regime that combines aerobic exercise and strength exercise. In addition, muscle stretching exercises are recommended for home training programs to increase muscle function.

코로나와 신체활동 저하

전 세계적으로 코로나19 사태로 인한 사회적 거리두기와 격리 조치가 신체활동을 현저하게 감소시키고 있다. 국제 설문조사를 분석한 결과, 코로나19 팬데믹(대유행)으로 집안에 머무르는 시간이 길어지면서 사람들의 신체활동이 35% 감소하고 앉아있는 시간이 28.6% 증가했다[1].

국내에서도 건강증진개발원이 2020년 10월 성인 1,031명을 대상으로 실시한 ‘코로나19 이후 생활의 변화’ 결과를 보면, 응답자의 32.9%가 “코로나19로 현재 충분한 신체활동을 하지 못하고 있다”고 답했고, 42.1%가 체중이 증가되었다고 답했다[2]. 연세대학교 국민건강증진연구소의 2021년 2월 성인 1,500명을 비대면(온라인)으로 조사한 결과에 의하면 코로나19로 현재 신체활동을 중단했다는 응답이 남성은 48.7%, 여성은 47.0%로 나타나 신체활동 감소 현상이 뚜렷하게 나타났다[3].

이와 같은 신체활동 저하는 골관절염, 고혈압, 고지혈증, 2형당뇨병과 같은 만성질환 악화 등 건강에 부정적인 영향을 미친다.

코로나19로 인한 신체활동의 감소는 연간 2형당뇨병 환자를 1,110만 명 이상 증가시키고 사망자를 170만 명 이상 증가시킬 수 있다는 연구결과가 발표되었다. 특히 신체활동이 없는 40대 이상이 당뇨병으로 발전할 확률은 57.3%였으며 당뇨병 위험이 있는 사람들의 경우에는 57.7%였다[1].

당뇨병 환자의 운동 효과

운동은 당뇨병과 관련성이 높다. 규칙적인 신체활동이 당뇨병과 합병증을 예방하고 지연시킬 수 있으며, 2019년에 발생한 3,300만여 명의 2형당뇨병 환자는 신체활동의 부족이 중요한 원인으로 작용했고, 2018년에 발생한 사망자 530여 명도 신체활동의 부족이 중요한 원인으로 작용했다는 사실이 몇몇 연구를 통해 확인되었다.

일반적으로 운동을 통해 근육이 수축할 때 혈중 포도당의 흡수가 증가되는데, 간에서의 글리코겐분해와 포도당신생성, 유리지방산과 같은 대체 연료에 의해서 혈당은 일정하게 유지되는 효과가 있다[4,5]. 유산소운동의 효과는 인슐린민감성을 개선시킬 수 있고, 포도당 대사와 인슐린에 대한 근육의 반응을 개선시켜서 근육 내 지방저장과 지방 산화능력을 증대시킨다[68]. 또한 저항성운동의 효과는 혈당을 개선시키고 인슐린 작용을 증가시켜 인슐린저항성을 개선시킨다[9,10].

운동은 코로나19와 관련이 있는 것으로 보인다. 코로나19 확진자가 운동이 부족하면 입원위험은 2.2배, 사망위험은 2.5배 높아지며, 운동이 코로나19 치료에 효과가 있다[11]. 운동을 했을 때 근육에서 분비되는 호르몬 아이리신(irisin)이 코로나19 바이러스가 신체에 침투하는 데 쓰이는 수용체 ACE-2 (angiotensin-converting enzyme 2) 유전자를 억제하는 것으로 보고되었다[12].

당뇨병 환자의 운동 방법

미국당뇨병학회(American Diabetes Association, ADA)와 미국스포츠의학회(American College of Sports Medicine, ACSM)에서는 2형당뇨병 환자의 경우 유산소운동, 저항성운동, 유연성 및 평형성운동을 병행하도록 권장하고 있다. 1회 운동 시 중등도 강도의 운동을 30분 이상씩, 가능한 한 일주일 내내 실시하는 것이 이상적이며, 가능하다면 매일 운동하기를 권하지만 어려운 경우 1회 운동 시간을 더 늘릴 수 있다[13,14].

코로나19 대유행 시대에는 실내 운동과 야외 운동에 제한이 있으므로 당뇨병 환자의 건강한 삶을 영위하기 위한 운동으로 집에서 실천할 수 있는 홈트레이닝 프로그램을 활용하는 방안을 추천한다.

코로나 시대, 당뇨병 환자의 홈트레이닝

코로나 시대 당뇨병 환자를 위한 홈트레이닝의 목적은 집에 머무는 시간이 많아지므로 발생한 체지방률 증가를 개선하고, 코로나 바이러스 생성 억제와 인슐린저항성 개선을 위한 근육의 질적·양적 관리이다.

체지방률 개선을 위해서는 유산소운동이 권장되며 근육량 증가를 위해서는 근력운동이 추천되므로 홈트레이닝 프로그램은 유산소운동과 근력운동이 복합적으로 구성된 순환운동을 실시할 것을 권장한다. 또한 근육의 질적 개선을 위한 홈트레이닝 프로그램으로 유연성운동을 실시할 것을 권장하며, 개인의 체력 수준에 따라 반복 횟수를 정해서 실천하기를 바란다.

홈트레이닝 프로그램을 실시할 때는 요가 매트 위에서 실시하는 것이 좋으며 가능하면 바른 자세를 만드는 데 도움을 주는 번호가 새겨진 매트를 사용하면 좋겠다.

요가 매트 위에서 8가지 동작을 순환하면서 실시하는 서킷 트레이닝으로 구성되었으며, 혼자서 또는 여러 명이 함께 실시할 수 있다.

프로그램 1회당 40분(준비운동 5분 + 본운동 30분 + 정리운동 5분)으로 구성되어 있으며, 본운동 30분 프로그램은 1동작당 45초간 운동 실시, 휴식 및 다음 동작 준비 15초 실시, 운동프로그램 8분 후 2분 동안 휴식하고 총 3회 반복한다. 개인의 체력 수준과 적응 정도에 따라 10분 운동을 1회, 2회, 3회 가능하다(Supplementary Table 1, Supplementary Figs. 1, 2).
