Trans Abstract

Diabetes technology that includes artificial pancreas system (APS) has achieved remarkable progress in the past decades. The first APS, the Medtronic MiniMed 670G system introduced in 2017, and other subsequent commercial APSs have come as answers to patient and clinician prayers. However, these devices are not accessible by most persons with type 1 diabetes (T1D) in Korea. Therefore, highly motivated, and tech-savvy persons in the diabetes community have started developing open source APS (Open APS) that integrate continuous glucose monitoring (CGM), insulin pumps, and smartphone technology to run their own do-it-yourself APS (DIYAPS). Observational studies have revealed significant improvements in HbA1c levels, time in range, and quality of life after the initiation of Open APS use. However, the use of regularized CGM and an insulin pump with an unauthorized DIYAPS makes these devices off-label and this has been a matter of grave clinical concern. In addition, lack of randomized controlled trials, insurance coverage, and legal issues are barriers to the general acceptance of DIYAPS among persons with T1D. In this article, I will summarize existing DIYAPS studies including those in Korea and their concerns. In addition, I will introduce new insulin pumps available in Korea.

서론

1형 당뇨병은 만성합병증 예방을 위해서는 하루 7∼10회 이상의 자가혈당측정 혹은 연속혈당측정기와 다회 인슐린 주사 혹은 인슐린 펌프를 사용한 적극적인 혈당 조절이 필요하지만 중증저혈당 위험성으로 목표 혈당에 도달하거나 지속적으로 유지하기가 어려운 실정이다[1]. 특히 스트레스, 운동, 활동량 변화, 소화 능력의 변화, 다양한 식사 등 무수한 변수가 발생하는 일상생활에서 필요한 만큼의 정확한 인슐린 용량을 예측하여 정확한 시간에 투여하는 것은 이론처럼 쉽지 않다. 특히 밤 시간 동안 수면 중에 일어나는 혈당 변화를 지속적으로 관찰하여 기저 인슐린 용량을 실시간 변경하는 것은 본인 스스로 하기에는 매우 어려운 일이다. 국내에서 1형 당뇨병은 소아청소년에서 발병 빈도가 증가하지만 성인에서도 지속적으로 발병하고 있으며, 상대적으로 천천히 진행하는 비전형적인 1형 당뇨병은 고령 인구에서 많이 발생한다[2]. 전체 1형 당뇨인 유병률은 성인 비율이 75% 이상이고, 국내 1형 당뇨인 절반 이상에서 대사증후군을 동반하고 있다[2]. 특히 일반인이나 2형 당뇨병에 비해 국내 1형 당뇨병에서 심혈관합병증과 신부전의 발생 비율이 높은 것으로 보고되어 국내에서도 1형 당뇨병 치료가 잘 되지 않고 있음을 시사한다[3,4]. 2000년 이후 연속혈당측정기와 인슐린 펌프 및 인공췌장 기술이 급격하게 발전하고 있다. 2006년 센서연동형 인슐린 펌프가 나왔고, 2017년에 하이브리드 인공췌장 MiniMed 670G pump (Medtronic, Minneapolis, MN, USA)가 미국 식품의약국(U.S. Food and Drug Administration, FDA)의 허가를 처음 받았으며, Tandem Control IQ (Tandem Diabetes Care, San Diego, CA, USA)가 2020년에 FDA 허가를 받았다. 유럽에서는 DBLG1 system (Diabeloop, Paris, France), the DANA RS pump compatible-CamAPS FX app (Advance Therapeutics, Warwick, UK) 등이 허가된 상황이다. 하지만 국내에서는 아직 허가된 하이브리드 인공췌장 제품이 없다. 단지, 저혈당을 예측하여 기저인슐린 주입이 멈추는 기능이 있는 Medtronic의 MiniMed 640G pump만 사용되고 있어서, 오픈 소스 인공췌장 시스템(open source artificial pancreas system, do-it-yourself artificial pancreas system)을 사용하는 1형 당뇨인들이 최근 들어 늘어나고 있다.

오픈 소스 인공췌장 시스템

인공췌장을 포함한 최신 기기들이 임상 시험을 통해 정식으로 허가되는 과정이 힘들고 오랜 시간이 걸려서 1형 당뇨인 혹은 가족들이 스스로 관련 기기와 앱을 개발할 필요가 생겨났다. 2013년, ‘We are not waiting’이라는 문구를 가지고 온라인에 1형 당뇨인과 가족 및 관련 의료인들이 모여서 오픈 소스 프로그램과 기기에 대한 지식을 공유하기 시작하면서, 스마트워치로부터 연속혈당측정 데이터 모니터하기, 원격으로 아이들 혈당을 추적하기, 알람 기능 삽입 등의 환자 주도 프로젝트가 시작되었다[5]. 2014년 Dana Lewis, Scott Leibrand, Ben West 등은 오픈 소스 인공췌장 프로젝트를 시작하였고, 2015년 오픈 소스 인공췌장이 시작되었으며, 2020년 1월 기준 전 세계적으로 1,776명 이상에서 사용하고 있다.

오픈 소스 인공췌장은 실시간 연속혈당측정기 혹은 FreeStyle Libre에 트랜스미터(MiaoMiao, Blucon)를 결합한 형태와 이로부터 연속혈당측정기 데이터를 받아서 스마트폰에 보여주고 클라우드 저장소인 Nightscout에 데이터를 전송하는 앱(Xdrip 혹은 Spike), 인슐린 펌프, 인공췌장 알고리즘이 운용되는 미니 컴퓨터 혹은 스마트폰 앱으로 구성된다(Fig. 1). Table 1과 같이 3가지 형태의 오픈 소스 인공췌장이 전 세계적으로 운용되고 있으며, 국내에서는 Android APS와 다나 펌프, Dexcom G5 혹은 Libre (MiaoMiao 혹은 Bluecon) 조합이 가장 많이 이용되고 있다. 또한 소아 1형 당뇨 아이를 둔 부모들이 원격으로 실시간 모니터링하면서 문자메시지를 이용하여 아이들의 펌프에서 볼루스(bolus) 인슐린을 주입할 수 있는 기능(SMS commands)을 오픈 소스 인공췌장에서 이용하고 있다.

오픈 소스 인공췌장에서 이용하는 초기 알고리즘은 oref0 (Open APS Reference Design Zero)로 임시 기저인슐린(temporary basal) 기능을 이용해서 연속혈당측정 데이터를 분석하여 목표혈당범위에 벗어나는 혈당이 관찰되거나 예측되면 설정된 기저 인슐린에서 인슐린 용량을 증감하여 혈당을 안정적으로 만든다. 이는 야간 시간 동안과 식사와 식사 사이 혈당을 안정적으로 유지하는 데 매우 유용하다. 또한 사용자의 안전을 위해서 여러 기능이 설정되어 있다. 먼저 연속혈당측정기 데이터를 5분마다 읽고 인슐린 펌프의 최근 세팅, 즉, 현재와 최대 기저인슐린 주입속도, 최근 볼루스 주입량, 남아있는 활성 인슐린, 인슐린 민감도 지수, 인슐린 탄수화물 비, 인슐린 지속시간, 목표 혈당값 및 범위 등을 분석한다. 이를 가지고 볼루스 계산기를 업데이트하고 임시기저를 지속할지와 중단할지를 결정한다. 안전상의 이유로 오픈 소스 인공췌장은 30분 이상 펌프와 연동이 안되면 임시기저 인슐린은 멈추고 펌프에 설정되어 있던 기저인슐린 용량으로 복구하도록 설정되어 있다. 또한 인슐린 민감도 지수와 인슐린 탄수화물 비를 적절하게 보정하도록 추천하는 기능이 있는데 8∼24시간 데이터를 이용하는 Autosensitivity와 24시간 이상 혹은 사용자가 설정한 기간 동안 데이터를 이용하여 인슐린 민감도 지수, 인슐린 탄수화물 비, 기저인슐린 용량을 독립적으로 적절하게 추천해 주는 Autotune 기능이 있다. 최근에는 개인별 음식물 섭취 후 소화 속도 차이와 다양한 식사 종류에 따른 식후 혈당을 좀 더 효율적으로 조절하기 위해서 ‘Advanced Meal Assist (AMA)’ 기능이 추가되었다. 또한 업데이트된 oref1 알고리즘은 탄수화물 위주 식사 이후에 급속하게 올라가는 혈당과 식후 3∼4시간에 발생하는 저혈당을 조정하기 위해서 소량의 인슐린을 식전 및 식사 직후에 소량씩 나누어 주입하는 supermicroboluses (SMB) 기능이 추가되었다.

오픈 소스 인공췌장의 임상적 효용성은 여러 관찰 연구에서 당화혈색소와 70∼180 mg/dL time in range (TIR)를 10% 정도 개선하고, 저혈당과 고혈당 시간을 줄이고, 야간 저혈당 빈도가 줄어들고, 삶의 질을 개선하는 것으로 보고된 바 있으며, 국내에서 시행된 연구에서도 비슷한 결과를 보고한 바 있다[6,7]. 하지만 오픈 소스 인공췌장을 사용하 더라도 개인별로 70∼180 mg/dL TIR에 차이가 많이 관찰되는데, 이는 탄수화물 계량을 정확히 해서 볼루스 계산기를 잘 사용하는 경우에는 그렇지 않은 1형 당뇨인들에 비해서 오픈 소스 인공췌장 사용 후 70∼180 mg/dL TIR을 포함한 연속혈당측정 데이터가 더 좋은 것으로 생각된다. 즉, 볼루스 주입까지 자동으로 조절되는 완전한 인공췌장으로 진화하기 전에는 오픈 소스 인공췌장을 사용하는 경우에도 탄수화물 계량과 볼루스 계산기에 대한 교육이 중요함을 시사한다.

오픈 소스 인공췌장은 아직도 안전성 및 효능을 제대로 평가할 수 있는 무작위대조군 연구가 없으며 공식적으로 허가된 제품이 아니어서 아직까지도 1형 당뇨인 스스로 법적 책임을 가지고 사용해야 하는 문제점들이 있다. 2019년에 Open APS와 Medtronic 구형 펌프를 사용하다가 안전상의 문제가 발생하여 FDA가 경고문을 발표한 바 있다[8]. 즉, 동기 부여가 충분히 되어 있고, 스스로 온라인에서 사용법과 공개된 프로그램을 통해 연동 가능한 연속혈당측정기와 인슐린 펌프를 구입하여 스스로 사용할 수 있는 1형 당뇨인들이 주로 이용하고 있다. 초기에는 사용자가 적었지만 최근 들어 사용자가 점점 더 늘어나고 있으며 국내에서도 2020년 기준 200여 명 이상의 사용자가 있다. 따라서 외래를 방문하는 일반적인 1형 당뇨인의 경우에는 오픈 소스 인공췌장을 아직 권유하기는 어렵지만 이미 스스로 사용하고 있는 경우에는 의료진들이 적극적으로 잘 사용하고 있는지 중재할 필요가 있다[9,10]. 미국과 유럽에서는 오픈 소스 인공췌장 사용자에 대한 대규모 관찰연구가 진행되고 있으며 FDA가 허가한 오픈 소스 인공췌장을 만들기 위해 ‘Tidepool Loop’ 프로젝트가 시작되었는데, iOS 기반 오픈 소스 인공췌장 앱을 준비하고 있으며 JDRF와 Helmsley Charitable Trust에서 후원하고 있다[10].

한국에서 개발되는 새로운 인슐린 펌프

국내에서도 최근에 자가혈당측정기와 연속혈당측정기 인슐린 펌프가 스마트폰 앱을 통해 연속혈당측정기 데이터 와 인슐린 펌프 데이터가 동시에 모니터링되는 앱이 개발되고, 모니터링 플랫폼을 통해서 진료실에서 인슐린 펌프와 연속혈당측정기 데이터를 동시에 분석할 수 있는 제품들(DIA:CONN G8 pump, CARE APP, CARE Web 등; G2e Information & Technology, Seoul, Korea)을 최근 서비스하기 시작하였다. 볼루스 계산기 기능에 연속혈당측정기 트렌드 화살표에 의한 혈당 변화 예측값을 반영 가능하고, 저혈당 발생 혹은 예방을 위한 탄수화물 양을 계산할 수 있는 기능을 추가하였다(Fig. 2). 또한 Insulet의 OmniPod 와 같은 패치형 펌프가 국내에서 EOPatch (EOFLOW, Seongnam, Korea)라는 제품으로 개발되었고, 최근 국내에서 허가되었다. 이 제품은 EOBridge 앱으로 스마트폰과 개인 컴퓨터에서 펌프 데이터를 모니터링 할 수 있다. 조만간 Dexcom G6와 연동하여 동시에 스마트폰 앱과 개 인 컴퓨터에서 모니터링 가능한 패치펌프 제품이 출시 예정이고, 미국의 Tandem Control IQ 인공췌장 제품에서 사용하는 Type Zero 인공췌장 알고리즘을 탑재한 패치형 펌프 제품에 대한 허가 임상 개시가 2021년 예정되어 있음을 EOFLOW에서 언론에 발표한 바 있으며, 연속혈당측정기와 패치펌프의 일체형 모델을 개발 중이다(Fig. 3) [11].

결론

결론적으로 아직까지 국내에서는 인공췌장 알고리즘을 탑재한 펌프가 허가된 제품이 없어서 1형 당뇨인 스스로 사용하는 오픈 소스 인공췌장 이용자가 늘어나고 있다. 여러 관찰 연구에서 오픈 소스 인공췌장 사용자들의 혈당이 개선되고 저혈당 고혈당 빈도가 줄어들고 삶의 질이 개선됨이 보고되었으나, 아직 무작위 대조군 연구를 통한 안전성과 효능이 입증되지 않아 식품의약품안전처에서 허가한 제품은 아직까지 없는 실정이다. 하지만 앞으로 정식으로 허가된 인공췌장 제품이 국내에서도 조만간 발매될 것으로 기대된다.
