Trans Abstract

Abstract

Increased blood pressure variability (BPV) has been found to significantly increase the risk of cardiovascular morbidity and mortality in the non-diabetic population. The prognostic importance of increased BPV for the development of micro- and macrovascular complications in type 2 diabetes has been investigated. Type 2 diabetes can increase BPV through different mechanisms, including increased arterial stiffness and autonomic dysfunction. Several studies in type 2 diabetic patients showed inconclusive results regarding all-cause mortality, microvascular complication, and extended major adverse cardiovascular events (MACE). However, some trials showed that BPV might be regarded as a potentially important therapeutic target in the management of type 2 diabetes regarding MACE. For now, the primary goal of antihypertensive treatment in patients with type 2 diabetes shall remain to controll mean blood pressure levels. However, when future studies show consistent results, BPV might be regarded as a therapeutic target in type 2 diabetic patients.

서론

45세에서 75세 사이의 2형당뇨병 환자들 중 약 40~60%는 고혈압이 있다고 추정된다[1]. 당뇨병 환자에서 고혈압은 심혈관합병증과 사망을 야기하는 주된 원인이다[2]. 또한 당뇨병 환자에서 혈압조절은 혈당조절보다 더 어렵다고 알려져 있다[3]. 이전에 여러 연구들에서 당뇨병 환자에서 고혈압을 조절하는 것이 심혈관합병증과 신장합병증을 감소시킬 수 있음이 밝혀져 왔다[4,5]. 하지만 최근 들어 혈압 그 자체뿐만 아니라 혈압변동성이 당뇨병 환자에서 합병증을 일으키는 요인으로 대두되고 있다.

2010년, Rothwell 등[6]이 처음으로 수축기혈압(systolic blood pressure)변동성이 일과성허혈발작이 있었거나 치료 중인 고혈압 환자에서 뇌졸중과 높은 관련성이 있음을 입증한 이래, 여러 연구자들이 혈압변동성이 인체에 미치는 영향에 대해서 조사하였다[79]. 현재 일반인에서 시행된 여러 선행연구에서는 혈압변동성이 평균혈압에 관계없이 총사망률과 심혈관질환의 예측인자라는 것이 밝혀졌다[79].

혈압변동성은 특히 2형당뇨병 환자에서 다양한 기전을 통해 증가될 수 있다[1013]. 2형당뇨병이 야기하는 죽상경화증과 자율신경계 조절 능력의 감소는 혈압변동성을 일으키는 주된 요인으로 생각되고 있다. 혈압변동성을 일으키는 죽상경화증은 대혈관합병증 및 미세혈관합병증의 발병과 관련되었으며 당뇨병 환자와 일반 인구 모두에서 사망률의 독립적인 예측인자이다[1416]. 일반 인구와 달리 2형당뇨병 환자는 심혈관질환 및 미세혈관질환 발병 위험이 높은 것으로 잘 알려져 있는데 이러한 합병증들과 혈압변동성과의 연관성을 확인해 보는 것이 필요하다. 이미 여러 선행연구들에서 2형당뇨병 환자에서 높은 혈압변동성과 미세혈관 및 대혈관합병증 및 사망률 사이의 연관성을 조사하였다[1729]. 이전에 많은 연구에서 혈압변동성이 2형당뇨병 환자에서 합병증과 모든 사망률의 위험인자일 수 있음을 보고하였으나 또 한편으로는 그 결과를 부정하는 연구 결과들도 발표되었다.

현재 혈압조절이 2형당뇨병 환자에서 심혈관합병증에 미치는 영향은 명백하게 증명되었다. 혈압변동성이 당뇨병의 심혈관합병증에 미치는 영향이 밝혀진다면 새로운 조절인자로서 임상에서 사용할 수 있을 것으로 보인다. 본 시론에서는 2형당뇨병 환자에서 혈압변동성의 당뇨병합병증 예측인자로서 의 가능성을 확인하고 그 병태 기전을 알아보려고 한다. 혈압변동성이 미치는 영향을 (a) 총사망률, (b) major adverse cardiovascular events (MACE; 심혈관계 사망, 비치명적 심근경색증 및 비치명적 뇌졸중), (c) extended MACE (심혈관계 사망, 비치명적 심근경색증, 치명적 및 비치명적 뇌졸중 및 말초동맥질환) 및 (d) 미세혈관합병증(신경병증, 신장병증/미세알부민뇨증의 발병, 망막병증)으로 나누어서 살펴보고자 한다.

본론

1. 수축기 및 이완기혈압변동성

Cardoso 등[30]은 2020년에 2형당뇨병 환자 682명을 10년 이상 추적한 Rio de Janeiro cohort study를 통해 혈압변동성의 유용성에 대해서 살펴보았다. 그 결과 24개월 혈압변동성이 12개월 혈압변동성보다 MACE 발생에 더 연관이 있다고 보고하였다[30]. 또한 전체적으로 수축기혈압변동성이 이완기혈압변동성보다 더 MACE 발생에 악영향을 미쳤음을 보고하였다. 또한, 혈압변동성에 대해서 최초로 체계적 문헌연구와 메타분석을 시행한 Chiriacò 등[31]의 연구에서도 이완기혈압변동성은 심혈관합병증 위험을 예측하지 못하였다.

2. 혈압변동성과 총사망률

혈압변동성과 총사망률에 대해 21개의 선행연구를 메타분석한 Chiriacò 등[31]의 연구에 따르면 수축기혈압변동성을 표준편차(standard deviation, SD)로 평가했을 때 1–SD 증가당 위험비는 1.12 (95% 신뢰구간 1.04~1.21)였다. 이전 여러 선행연구에서도 2형당뇨병 환자의 수축기혈압변동성이 총사망률과 연관 있다고 보고되었다[17,20,26,29]. 2형당뇨병 환자에서 수축기혈압변동성이 총사망률의 예측인자임을 부정하는 연구 결과들에서도 하위 그룹 분석에서 고령자 또는 항고혈압제를 사용하지 않는 사람에서는 수축기혈압변동성이 총사망률의 예측인자로 분석되었다[18,27]. 그런데 Action in Diabetes and Vascular Disease: Preterax and Diamicron Modified Release Controlled Evaluation (ADVANCE) 연구에서는 수축기혈압변동성이 총사망률을 예측하긴 했으나 추적 관찰 기간이 길수록 관련 예측도가 약화되었다고 보고하였다(초기 2.4년 이후 사망률 위험비 1.29, 7.6년 연장된 추적 기간 동안 위험비 1.13) [17,29]. 기간 차이 때문인지 2020년에 발표된 연구 관찰자들의 추적 중앙값이 11.3년(최대 16년)인 Cardoso 등[30]의 연구에서는 혈압변동성과 총사망률의 연관성을 찾지 못하였다.

3. 혈압변동성과 MACE/extended MACE

MACE에 관해서는 ADVANCE 연구를 포함한 여러 선행연구들에서[2931] 혈압변동성의 예후적 가치를 입증하였다. 하지만 extended MACE에 관련해서는 연구들마다 상이한 결과를 나타내었다. Chiriacò 등[31]의 연구는 수축기혈압변동성이 extended MACE 발생에 대해서 위험비 1.08 (95% 신뢰구간 1.02~1.17)로 유의함을 보여주었다. 그 외에 유의성을 보여준 연구들도 있으나[17,20], McMullan 등[26]과 Hsieh 등[28]을 비롯한 다른 연구들[26,28,30]에서는 혈압변동성과 extended MACE와의 관련성은 부정되었다.

4. 혈압변동성과 미세혈관합병증

1) 당뇨병신장병증

혈압변동성과 미세혈관합병증과의 연관성을 조사한 연구들의 경우 데이터가 부족하고 연구는 주로 후향적으로 이루어졌다. ADVANCE-ON [29] 연구를 제외한 다른 모든 연구에서 수축기혈압변동성은 알부민뇨 진행에 연관성을 보이거나[22,23,25,32] 또는 신장기능 악화[19,21,23,24,26] 사이에 연관성이 있음을 보여주었다. 하지만 가장 최근 전향적으로 장기간 추적 관찰한 Cardoso 등[30]의 연구는 수축기혈압변동성과 알부민뇨 및 신장기능 악화 사이의 관련성을 부정하였다.

2) 당뇨병망막병증

당뇨병망막병증에 대한 연구들[17,22,23,30]은 혈압변동성과 관련성을 찾지 못했다. 오직 Sohn 등[24]의 연구에서 혈압변동성과 당뇨병망막병증 사이의 연관성을 보여주었으나 결과 분석에 백내장을 포함하여 오로지 당뇨병망막병증의 연관성을 보았다고 말하기는 힘들다.

3) 당뇨병신경병증

당뇨병신경병증 발병 또는 악화 역시 Cardoso 등[30]의 연구에서 장기 추적관찰 결과 그 연관성을 부정하였다. 하지만 Sohn 등[24]의 연구에서 수축기혈압변동성과 당뇨병신경병증 간의 관련성을 보여주었으나 이 연구에서는 당뇨병신경병증을 국제질병분류에 의해서 판정하였으며, 따라서 수근관증후군 같은 질환도 당뇨병신경병증에 포함시켜 그 연구 결과를 전적으로 신뢰하기는 힘들다.

5. 혈압변동성의 병태생리

높은 혈압변동성이 심혈관질환 이환을 야기하는 병태생리 기전은 아직 확실하지 않다. 혈압변동성은 중추 교감신경의 과잉 활동, 레닌-안지오텐신-알도스테론계의 변화, 죽상경화증의 증가, 혈관 활성 물질 생성의 증가와 같은 여러 기전과 관련이 있는 것으로 보인다. 특히 2형당뇨병에서 높은 혈압변동성은 미세혈관손상을 일으키는 것으로 생각된다. 혈압변동성이 일으키는 박동성 에너지가 혈관벽에 압력 손상을 일으키는 것으로 보이며 2형당뇨병이 야기하는 죽상경화증이 여기에 일조하는 것으로 생각된다[33,34]. 뿐만 아니라 당뇨병 환자에서 생기는 자율신경계 기능장애는 혈압변동성에 대해 혈관들이 적응하는 것을 방해하는 것으로 생각된다[35,36]. 혈압변동성이 대혈관합병증을 일으키는 기전은 미세혈관합병증을 일으키는 기전과 다를 수 있다. 대혈관에서는 특히 죽상경화증과 자율신경계 기능 장애의 조합으로 수축기혈압에 더 큰 변동이 발생하는 것으로 보인다[37,38].

결론

고혈압은 여전히 세계 최고의 사망 위험 요소이다[3941]. 많은 수의 2형당뇨병 환자들에서 고혈압이 있으며 고혈압 치료를 받고 있다. 당뇨병 환자에서는 죽상경화증 그리고 자율신경계 조절 능력 저하로 혈압변동성이 심하게 발생하는 것으로 보이며, 앞서 보았듯이 혈압변동성이 증가되면 미세혈관손상이 일어나고 이는 표적장기손상을 야기한다.

2형당뇨병 환자에서 체계적 문헌연구와 메타분석[31]을 통해 혈압변동성에 대해서 분석한 결과에 따르면 장기간 수축기혈압변동성은 평균혈압에 독립적으로 총사망률, MACE, extended MACE, 미세혈관합병증의 위험 증가와 관련이 있었다. 이는 Stevens 등[8]이 일반 인구 집단에서 관찰한 것과 동일한 결과이다. 하지만 2020년 발표된 2형당뇨병 환자들을 장기간 전향적으로 추적 관찰한 Cardoso 등[30]의 연구에서는 수축기혈압변동성이 MACE 발생과는 연관있으나 총사망률, extended MACE, 미세혈관합병증의 위험 증가와는 관련이 없음을 보고하였다.

이처럼 혈압변동성이 2형당뇨병 환자에서 일반적인 혈압 수치보다 심혈관합병증을 예측하는 데 핵심 연관 인자로서의 가능성은 아직 명백하지 않다. 현재 혈압변동성을 측정하는 표준 방법이 없기 때문에 연구 결과들이 불일치 하는 것일지도 모른다. 그 외 원인으로는 연구과정에서 환자들의 항고혈압 치료에 대한 다양한 순응도, 환경 및 심리적 조건들을 들 수 있으며[42,43], 병원 치료 방문 횟수, 방문 간격, 혈압 측정 방법(수동 또는 자동), 특히 다양한 연구 추적 관찰 기간이 일률적인 결과를 보여주지 않는 데 일조하고 있는 것으로 보인다[8,44,45].

표준적인 방법으로 측정한 혈압변동성을 이용하여 심혈관합병증 및 사망률에 혈압변동성이 미치는 영향에 대해 더 전향적인 연구 결과가 있기 전까지, 2형당뇨병 환자에서 치료 목표는 혈당조절과 더불어 현재 가이드라인대로 혈압 그 자체를 중점적으로 조절해야 할 것으로 보인다. 하지만 미래에 일률적인 결과를 보이는 데이터가 충분히 쌓인다면 내원할 당시의 혈압 수치만이 아니라 혈압변동성 또한 2형당뇨병 환자에서 심혈관합병증 관리의 목표가 될 가능성이 있다.
