Comparison of Direct Measurement of LDL Cholesterol with Calculation Using Friedewald`s Equation |

Abstract |
Background Type 2 diabetes mellitus is a significant risk factor for cardiovascular disease and is associated with cardiovascular morbidity and mortality. Patients who have coronary heart disease (CHD) or diabetes mellitus are recommended to lower plasma low density lipoprotein-cholesterol (LDL-C) levels to less than 100 mg/dL. Therefore, exact measurement of plasma LDL-C is essential for treating and preventing CHD. We compared directly measured LDL-C levels with the LDL-C levels calculated from the Friedewald`s equation in diabetic and non-diabetic subjects. Methods: Data were collected from the 33,792 samples who visited health care center at Asan medical center from January 2006 to November 2006. Total cholesterol and triglyceride levels were measured using traditional enzymatic methods. High density lipoprotein-cholesterol (HDL-C) and LDL-C were measured using a direct method that dose not require precipitation. LDL-C was again estimated by the Friedewald`s equation. Results: Among total subjects, the mean LDL-C levels were 122.2 ± 29.9 mg/dL and 107.4 ± 30.6 mg/dL estimated by direct measurement and calculated by Friedwald`s equation, respectively. On linear regression analysis, a strong correlation was found between two values (r = 0.973, P < 0.001). However, the Friedewlad`s equation underestimated the LDL-C in regard to the direct measurement, which was more pronounced in subjects with higher triglyceride levels. Conclusion: When compared with direct measurement, the Friedewald`s equation underestimated LDL-C levels, especially when plasma triglyceride levels increased. It is recommended to directly measure LDL-C levels in subjects with diabetes mellitus or with high triglyceride levels. |
