항암화학요법 중인 당뇨병환자의 식사요법

Abstract

Managing hyperglycemia during chemotherapy is challenging due to the metabolic shift caused by such treatments and the risk of malnutrition. This study aims to review the metabolic changes in these patients and propose effective nutritional strategies. Based on ESPEN (The European Society for Clinical Nutrition and Metabolism) guidelines and recent clinical studies, this paper analyzes the impact of chemotherapeutic agents (e.g., glucocorticoids, mTOR inhibitors) on glucose metabolism and discusses optimal intake levels for macronutrients and micronutrients. Nutritional priorities should focus on preventing malnutrition rather than overly strict glycemic control, which may cause hypoglycemia. Key recommendations include an energy intake of 25∼30 kcal/kg/day and protein intake of 1.0∼1.5 g/kg/day (up to 2.0 g/kg/day). Increasing the fat-to-carbohydrate ratio may be beneficial for patients with insulin resistance to improve energy density. Nutritional therapy for cancer patients with diabetes is a critical component of oncologic care that enhances treatment tolerance as well as survival. Personalized nutritional assessment and timely intervention are essential to balance glycemic control with optimal nutritional status.

서론

세계적으로 암과 당뇨병은 유병률이 증가하고 있다[1]. 당뇨병은 종양 환자에서 흔하게 동반되며, 인구 특성 및 암 유형에 따라 보고된 유병률은 7%에서 25%에 이른다[2]. 암 환자는 기저질환으로 당뇨병을 진단받지 않았더라도 새로 당뇨병이 생기거나 고혈당증이 나타날 위험이 크고, 당뇨병을 동반한 암 환자는 혈당조절이 잘 안되는 경우가 많다[3]. 암세포 자체의 대사적 변화와 더불어 항암화학요법 치료제 및 스테로이드 사용 등은 환자의 혈당을 급격히 상승시킬 수 있다[4,5]. 고혈당 조절을 위해 과도하게 섭취를 제한할 경우 암 환자의 영양불량과 근감소증(sarcopenia)이 심화할 수 있으며, 반대로 영양 보충에만 집중할 경우 고혈당으로 인한 감염 위험 증가 및 면역기능 저하를 초래할 수 있다[6]. 따라서 본 글에서는 항암화학요법 중인 당뇨병환자가 겪는 대사적 변화를 이해하고, 이를 극복하기 위한 식사요법의 원칙과 전략을 논의하고자 한다.

본론

1. 항암화학요법 중인 당뇨병환자의 대사적 특성

1) 항암화학요법 약물로 인한 고혈당 유발

항암화학요법 과정에서 사용되는 많은 약물은 혈당 대사에 직접적인 영향을 미친다. 글루코코티코이드(glucocorticoids)의 사용이 고혈당을 유발하거나 기존 당뇨병을 악화시킬 수 있다는 것은 널리 알려진 사실이다[7]. 구토 방지 및 식욕 개선, 부종 감소 등을 위해 흔히 처방되는 글루코코티코이드는 간의 포도당생성을 증가시키고, 말초 조직의 인슐린감수성을 낮추며 췌장의 인슐린분비를 억제하여 고혈당을 유발한다[4]. 특히 스테로이드로 인한 고혈당은 공복혈당보다 식후혈당을 더 급격히 상승시키는 특징이 있다[8]. 또한 표적치료제 중 mTOR 억제제(mammalian target of rapa-mycin inhibitors)인 에베롤리무스(everolimus)는 환자의 10∼50%에서 고혈당을 유발할 수 있으며, 중증 고혈당은 흔하지 않다. 인슐린신호전달 경로를 직접적으로 차단하여 2형당뇨병과 유사한 인슐린저항성을 유발한다[9]. 면역관문억제제(immune checkpoint inhibitors)인 PD-1/CTLA-4 억제제는 드물지만 췌장 베타세포를 자가면역 기전으로 파괴하여 1형당뇨병과 유사한 급성 인슐린의존성 당뇨병(또는 당뇨병케토산증)을 유발할 수 있다[10].

2) 항암화학요법 중 암 악액질(cachexia)과 근감소증

종양 자체 및 면역반응에서 분비되는 염증사이토카인(TNF-α, IL-6 등)은 전신 염증반응을 유발하여 지방분해와 근육 단백질분해를 촉진한다[11,12]. 이에 따라 환자는 적절한 영양과 인슐린이 공급됨에도 불구하고 근육량이 지속적으로 감소한다[12]. 또한 고혈당이 지속되면 만성염증과 대사이상을 동반하여 근육 단백질 합성 저하 및 분해를 촉진하는 경향이 있으며, 이에 따라 근감소증의 위험이 증가한다[13]. 항암화학요법 중 구역, 구토, 식욕부진, 설사 및 체중감소와 같은 부작용의 발생 빈도가 높으며, 이는 결국 영양불량과 근감소증으로 이어질 수 있다. 이 과정에서 골격근량이 감소하고 신체 기능이 저하되는 결과가 발생할 수 있다[14,15].

2. 당뇨병 동반 암 환자 식사요법의 원칙 및 섭취 전략

항암화학요법으로 인해 식사량이 불규칙한 환자에게 엄격한 혈당조절을 적용하는 것은 치명적인 영양불량 및 저혈당을 초래할 수 있으므로 고혈당을 예방하는 데 초점을 맞추는 것이 현실적이다[16].

1) 에너지

암 환자의 약 50%에서 휴식에너지소비량(resting energy expenditure, REE)이 예측치의 110% 이상으로 증가하는 대사항진을 보였다[17]. 다수의 암 환자에서 REE가 증가하지만 총에너지소비량(total energy expenditure, TEE)을 고려하면 건강한 사람의 TEE가 진행성 암 환자의 TEE보다 높게 나타났다. 암 환자에서 에너지소비는 신체활동 감소로 상쇄하므로 건강한 성인과 유사하게 TEE로 영양요구량을 산정한다[12]. TEE는 REE와 신체활동을 바탕으로 하거나 체중 등의 간단한 수식을 통해 산출할 수 있다. 암 환자의 TEE 는 개인차가 있지만, 유럽임상영양대사학회(The European Society for Clinical Nutrition and Metabolism, ESPEN)의 암 환자 가이드라인에 따르면 건강한 성인과 유사하게 체중 1 kg당 25∼30 kcal/day의 에너지섭취를 권고하고 있다[12,18]. 또한 ESPEN의 입원 환자 영양 가이드라인에 따르면 암 환자에게 에너지 공급을 우선하고, 체중 변화를 모니터링하며 필요시 에너지 밀도가 높은 음식과 음료를 도입하도록 권고한다[19].

2) 단백질

단백질 공급은 회복을 돕고 근육감소를 예방하는 데 필수적이다. 최소 1.0 g/kg/day 이상의 단백질이 필요하며, 가능한 경우 1.5 g/kg/day까지 늘릴 것을 권장한다[12]. 만약 극심한 전신 염증 및 동화 저항성(anabolic resistance)이 발생한 환자라면 체중 1 kg당 최대 2.0 g/day의 단백질 공급이 요구될 수 있다[20]. 신기능이 양호하다면 최대 2 g/kg/day 수준의 단백질 섭취는 안전하며, 급성신부전 환자에서는 1.0 g/kg/day, 만성신부전 환자의 경우 1.2 g/kg/day를 초과해서는 안 된다[12].

3) 탄수화물과 지방의 비율 조정

인슐린저항성이 동반된 암 환자는 근육의 포도당 이용이 저하되지만 지방 이용은 보존되므로 에너지대사 효율을 높이기 위해 탄수화물 대비 지방 비율을 높이는 영양 전략이 도움이 될 수 있다[21]. 이는 식사의 에너지 밀도를 높여 식욕이 저하된 환자가 적은 양으로도 충분한 열량을 섭취하도록 돕고, 식후혈당부하(glycemic load)를 줄이기 위함이다. 따라서 양질의 지방을 활용한 열량 보충은 긍적적이다. 반면에 케톤생성 식단(ketogenic diet)이나 단식은 영양불량 위험을 가중시킬 수 있고, 암 환자에게 케톤생성 식단이 유익하다는 것을 입증하는 임상시험은 아직 없기에 주의가 필요하다[12].

4) 미량영양소(비타민과 무기질)

ESPEN 가이드라인에 따르면 암 예방이나 치료 목적을 위한 비타민과 무기질은 권장섭취량(recommended dietary allowance, RDA)과 비슷한 양으로 섭취하는 것이 권고되며, 결핍이 없는 경우 고용량의 비타민 또는 무기질 보충제를 섭취하는 것은 권장되지 않는다. 고용량 항산화제(베타카로틴, 비타민 E 등)의 무분별한 복용은 오히려 항암화학요법의 효과를 방해하거나 일부 암종에서는 사망률을 증가시킬 수 있다는 연구 결과가 있다. 단, 혈액 검사로 확인된 결핍(예: 비타민 D 결핍)이 있거나 영양불량 상태의 환자에서는 일일 권장량 수준의 종합 비타민·무기질 보충이 안전하고 유익하다[12].

결론

당뇨병을 동반한 암 환자는 암 자체로 인한 대사 변화와 더불어 항암화학요법 과정에서 나타나는 고혈당 및 영양소 흡수 불량 등의 복합적인 상황에 놓이게 된다. 지속적인 체중감소와 근육감소는 생존율을 현저히 떨어뜨리므로 항암화학요법 중인 당뇨병환자에게 식사요법은 단순한 보조적 수단이 아니라 치료 효과 및 삶의 질 향상을 위한 필수 요소이다. 환자는 치료 시작 전 및 치료 기간 중 정기적인 영양평가를 받고, 적절한 시점에 개별화된 영양중재가 이루어져야 한다. 결론적으로 항암화학요법 중인 당뇨병환자의 효과적인 영양관리를 위해서는 치료 과정에서 발생하는 대사적 변화를 이해하고, 증상 및 상황을 고려한 전략적 접근이 필요하다.
