당뇨병망막병증 관리: 전신 당뇨병 치료를 통한 통합적 접근

Abstract

Diabetic retinopathy (DR) is one of the most common and vision-threatening complications of diabetes, affecting approximately one-third of patients. Systemic management of diabetes plays a crucial role in delaying the onset and progression of DR. Intensive glycemic control, as demonstrated in the DCCT (Diabetes Control and Complications Trial) and the UKPDS (United Kingdom Prospective Diabetes Study), significantly reduces long-term risk of retinopathy. However, rapid glucose lowering, particularly in patients with preexisting DR, may lead to an early worsening phenomenon. This transient deterioration is thought to be related to hypoglycemia, hemodynamic changes, and synergistic effects of insulin and vascular endothelial growth factor. In addition to glucose control, strict management of blood pressure and lipid profiles, including use of fenofibrate, further reduces the risk of progression. The role of glucagon-like peptide-1 receptor agonists in DR remains controversial, with early signals of worsening observed in SUSTAIN-6 but not consistently demonstrated in meta-analyses. Current evidence emphasizes the long-term benefits of early intensive systemic management, despite short-term risks. A patient-specific multifactorial management approach effectively reduces the risk of onset and progression of DR.

서론

당뇨병망막병증(diabetic retinopathy, DR)은 전 세계적으로 실명의 주요 원인 중 하나로 꼽히며, 특히 경제활동 인구에서 시력 상실을 초래하는 가장 흔한 질환 중 하나이다[1]. 전 세계 당뇨병환자의 약 3분의 1에서 망막병증이 발생하며, 그 중 약 10%는 시력 위협성 당뇨병망막병증(vision-threatening DR)으로 진행하는 것으로 보고된다[2]. 당뇨병망막병증의 발생 및 진행에는 혈당, 혈압, 지질 상태를 포함한 전신적 요인이 긴밀히 작용한다[3]. 따라서 효과적인 관리와 치료를 위해서는 내과 전문의와 안과 전문의 간의 긴밀한 협진이 필수적이다.
본 고에서는 당뇨병 전신관리와 망막병증의 연관성, 기존 대규모 임상연구의 결과, 조기 악화 현상(early worsening)의 병태생리, 혈당ㆍ혈압ㆍ지질 관리 전략, 그리고 최근 주목 받는 glucagon-like peptide-1 (GLP-1)수용체작용제의 망막병증에 대한 논란을 고찰하고자 한다.

본론

1. 당뇨병망막병증의 임상적 특성

당뇨병망막병증은 비증식성(non-proliferative DR) 단계와 증식성(proliferative DR) 단계로 나눌 수 있으며, 황반부종(macular edema)은 모든 단계에서 시력 저하의 중요한 원인이 된다. 안저 소견으로는 미세혈관류(microaneurysm), 점상 및 반점출혈(dot-and-blot hemorrhage), 면화반(cotton-wool spot), 경성삼출물(hard exudate), 망막내미세혈관이상(intraretinal microvascular abnormality), 그리고 염주정맥(venous beading) 등이 관찰된다[4].
당뇨병망막병증의 분류에는 Early Treatment Diabetic Retinopathy Study (ETDRS) 등급 시스템이 사용되며, 7장의 표준시야 입체사진을 활용하여 망막병증 관련 병변을 평가하고 미리 정해진 등급에 따라 분류한다. ETDRS 등급은 10단계부터 90단계까지의 점수로 구성되며 수치가 높을수록 중증도가 높음을 나타낸다. 2단계 또는 3단계 이상 증가할 경우 망막병증이 진행한 것으로 정의된다[4].

2. 혈당조절과 망막병증

1) 대규모 임상연구의 근거

Diabetes Control and Complications Trial (DCCT)은 1형당뇨병 환자를 대상으로 한 대표적 연구로, 집중 인슐린치료군에서 당뇨병망막병증 발생 위험(ETDRS 등급 3단계 이상)이 76% 감소하고 진행 위험(ETDRS 등급 3단계 이상)은 54% 감소하였다[5]. 9년 추적 관찰에서 집중 치료의 효과가 뚜렷했으며, 이후 Epidemiology of Diabetes Interventions and Complications (EDIC) 연구에서 최대 18년까지 장기적인 보호 효과가 지속됨이 확인되었다[6].
2형당뇨병에서는 United Kingdom Prospective Diabetes Study (UKPDS)가 대표적이다[7]. 신규 진단 환자를 대상으로 혈당조절 강화를 시행한 결과, 9년 시점부터 ETDRS 등급 2단계 이상 진행 위험이 유의하게 감소하였다. 이후 ACCORD, ADVANCE, VADT 연구를 포함한 메타분석에서도 당화혈색소를 평균 0.9% 낮추는 것이 5년 동안 망막 사건을 유의하게 줄이는 것으로 나타났다[8].

2) 조기 악화 현상

흥미롭게도 일부 연구에서는 혈당조절을 갑작스럽게 강화할 경우 단기적으로 망막병증이 악화되는 조기 악화 현상이 관찰되었다. DCCT 연구에서도 기저 망막병증이 있었고 당화혈색소가 급격히 2% 이상 감소한 환자에서는 초기 1∼2년 동안 망막병증 악화가 증가하는 현상이 관찰되었다[5]. 조기 악화와 관련된 주요 위험인자로는 초기 높은 당화혈색소 수치, 첫 6개월 동안의 빠른 당화혈색소의 감소, 그리고 기저 망막병증 유병이 포함된다[9]. 췌장이식, 대사수술, 임신군에서도 망막병증의 조기 악화 위험인자가 유사한 양상을 보였다[10-12].
조기 악화의 가능한 병태생리 기전은 다음과 같다. 첫째, 저혈당에 의한 허혈성 손상이 망막조직 악화를 초래할 수 있다[13]. 둘째, 혈장포도당 농도의 급감은 혈관 내 삼투압을 낮춰 망막 소혈관 손상을 유발할 수 있다[14]. 마지막으로, 고용량 외인성 인슐린이 혈관내피성장인자(vascular endothelial growth factor, VEGF)와 상승작용해 혈관증식 및 망막병증 악화를 일으킬 수 있다[15].
그러나 DCCT/EDIC 및 UKPDS 연구를 통해 장기간에 걸친 집중적 혈당조절이 당뇨병망막병증의 진행을 효과적으로 억제하는 것으로 일관되게 확인되었다[5,6]. 즉, 장기적인 관점에서 볼 때 집중적 혈당조절로 인한 전반적인 이득은 단기적 위험(조기 악화 현상)을 상회하는 것으로 보인다.

3. 혈압 조절과 망막병증

UKPDS 연구에 따르면, 고혈압과 2형당뇨병을 동시에 가진 환자에서 엄격한 혈압 조절은 당뇨병망막병증의 발생을 지연시키고 시력 저하 위험을 줄이는 효과가 있었다[16]. 특히 기저 망막병증의 유무와 관계없이, 7.5년에 이르는 장기 추적 결과에서 엄격한 혈압 조절은 면화반 발생과 ETDRS 등급 2단계 이상 진행 위험을 모두 감소시키는 것으로 나타났다[17]. 반면, ACCORD BP 및 ADVANCE BP 연구에서는 강력한 혈압 조절이 망막병증에 대해 중립적인 효과를 보였는데[18,19], 이는 연구에 참여한 환자의 특성이나 혈압 조절 목표치, 그리고 추적 기간과 같은 요소들의 차이에서 기인한 결과들로 해석된다.

4. 지질 조절과 망막병증

혈중 지질 이상은 경성 삼출물 및 황반부종 발생과 밀접한 관련이 있다[20]. FIELD 연구에서는 fenofibrate를 투여했을 때 당뇨병망막병증의 진행이 30% 감소하는 효과가 관찰되었으며[21], 이 효과는 LDL (low density lipoprotein)콜레스테롤 감소와는 별개로 나타났다. 또한 ACCORD Eye 연구에서는 simvastatin 단독투여에 비해 fenofibrate 병용투여가 당뇨병망막병증의 진행을 40% 감소시키는 결과를 보였다[19]. 이러한 근거를 바탕으로, 지질 조절은 혈당 및 혈압 조절과 함께 당뇨병망막병증 예방 및 관리를 위한 다중 위험인자 관리의 중요한 축임을 알 수 있다.

5. GLP-1수용체작용제와 망막병증

최근 2형당뇨병 치료에서 GLP-1수용체작용제는 심혈관 및 신장 보호 효과가 입증되면서 임상 현장에서 널리 사용되고 있다[22]. 그러나 당뇨병망막병증과 관련해서는 아직 논란이 존재한다.
LEADER 연구에서는 리라글루타이드 투여군에서 망막병증 사건이 다소 많았으나, 통계적으로 유의한 차이는 없었다[23]. 반면 SUSTAIN-6 연구에서는 세마글루타이드 투여군에서 망막병증 합병증이 유의하게 증가하였으며[24], 특히 기저에 망막병증이 있던 환자와 당화혈색소가 2% 이상 급격히 감소한 환자에서 위험이 높게 나타났다[25]. 이러한 결과에 대해 해석의 한계가 존재하는데, 해당 연구들은 당뇨병망막병증 평가에 있어서 ETDRS 등급 시스템을 체계적으로 사용하지 않고 연구자 보고에 의존했기 때문이다[25]. 이후 진행된 메타분석에서는 GLP-1수용체작용제가 당뇨병망막병증 위험을 유의하게 증가시키지 않는 것으로 보고되었으며[26,27], 동물 연구에서는 망막신경 보호 효과도 확인된 바 있다[28]. 현재 진행 중인 FOCUS 연구(2027년 완료 예정)를 통해 GLP-1수용체작용제와 당뇨병망막병증의 연관성에 대한 보다 명확한 근거가 제시될 것으로 기대된다[29].

결론

당뇨병망막병증은 혈당조절 불량, 고혈압, 지질 이상 등 다양한 전신 대사 이상이 복합적으로 작용하여 발생하고 진행하는 질환이다. DCCT, UKPDS, FIELD 등 대규모 연구에서는 혈당, 혈압, 그리고 지질 조절의 중요성을 일관되게 강조하였으며, 특히 집중적인 혈당조절이 장기적인 관점에서 당뇨병망막병증의 발생과 진행을 효과적으로 억제한다는 점이 확인되었다.
이에 따라 실제 임상 현장에서는 내과와 안과의 협력적 접근을 통해 환자의 전신 건강 상태를 종합적으로 평가하는 것이 필수적이다. 환자 개개인에게 맞춘 다중 인자 관리 전략을 적용함으로써, 당뇨병망막병증의 발생과 진행 위험을 최소화할 수 있겠다.
