노인당뇨병 환자의 임상영양치료

Abstract

Type 2 diabetes mellitus becomes increasingly more common with age. As such, older adults represent a very heterogeneous group in terms of functional ability and cognitive status. Older adults with diabetes are distinct from younger adults with diabetes, leading to differences in therapeutic approach. Some important goals of nutrition therapy in older adults with diabetes include minimizing hypoglycemia and hyperglycemia and meeting individualized glycemic goals. Nutrition therapy for older adults with diabetes should include meeting the nutritional needs for patients while considering their age, evaluating their fluid intake, avoiding significant weight loss, and being sensitive to individual preferences and long standing food habits while advocating good nutrition. However, there is little evidence for nutritional guidelines for older adults with diabetes, thus more research is needed.

서론

2020년 통계청의 고령자 통계에 의하면 2020년 65세 이상 고령인구는 우리나라 인구의 15.7%로, 향후에도 계속 증가하여 2025년에는 20.3%에 이르러 우리나라가 초고령사회로 진입할 것으로 전망되고 있다. 2019년 65세 이상 고령자의 사망원인은 암(750.5명), 심장질환(335.7명), 폐렴(283.1명), 뇌혈관질환(232.0명), 당뇨병(87.1명) 순서로 나타났고[1], 대한당뇨병학회 Diabetes Fact Sheet in Korea 2020에 따르면 65세 이상 성인에서는 약 10명 중 3명(27.6%)이 당뇨병으로 조사되었다[2]. 노인당뇨병 환자의 혈당조절 목표는 신체기능, 인지기능과 기대여명을 평가하여 개별화하도록 권고하되, 노인당뇨병 환자에게 운동과 임상영양치료를 포함한 교육은 혈당조절과 삶의 질 향상에 도움이 되는 것으로 알려져 있다[3]. 따라서 본 원고에서는 노인당뇨병 환자에 있어 고려해야 할 임상영양치료에 대해 알아보고자 한다.

본론

1. 노인당뇨병 환자의 혈당조절 목표

노인당뇨병 환자를 대상으로 한 연구와 높은 수준의 진료지침 근거가 부족한 상황에서 전반적인 건강상태를 포괄하는 3단계 범주에 따른 혈당조절 목표가 제안되고 있다[4]. 건강하고 기대여명이 긴 노인의 경우 당화혈색소 목표를 7.0~7.5%로, 일반적인 노인의 경우 동반질환, 신체활동, 인지기능에 따라 7.5~8.5% 범위 내로, 전반적인 건강상태가 좋지 못하고 기대여명이 짧은 경우 당화혈색소 목표 기준 대신 저혈당 회피와 고혈당으로 인한 급성 합병증 조절에 중점을 두도록 권고하고 있다(Table 1) [5-7].

대한당뇨병학회에서는 노인당뇨병 환자의 혈당조절 목표에 대해 일반적으로 당화혈색소 7.5% 미만이지만, 기대여명과 노쇠의 정도를 고려해 개별화하도록 제안하고 있다[8].

2. 노인당뇨병 환자의 임상영양치료

노인당뇨병 환자에게서 운동과 임상영양치료는 도움이 되는 것으로 알려져 있다[4]. 최근 다년간의 임상연구 결과를 살펴보면 노인당뇨병 환자를 위한 다량영양소와 미량영양소의 조정에 대한 구체적인 권고사항은 없다[9]. 일반적으로 노인당뇨병 환자를 위한 영양권고사항들은 성인당뇨병 환자 기준에 기반을 두고 전반적인 영양소 필요량은 특정 연구결과보다 임상적 판단에 근거하도록 제시하고 있다[10].

1) 에너지와 체중

일반적으로 노화가 진행될수록 다양한 종류의 식품과 섭취량이 감소하게 되므로, 노인당뇨병 환자의 영양소 필요량을 충족시키는 충분한 식사 계획이 무엇보다 먼저 필요하다. 보다 정확한 에너지 요구량 산정을 위해서 연령이 포함된 에너지 산출 공식을 사용하고, 체중 변화 이력과 가능하다면 체중 이외 체지방, 체근육 등의 신체계측치를 확인한다[10]. 성인당뇨병 환자에게 적용되는 체중관리 전략과는 다르게 노인에 있어서 이환율과 사망률의 증가에 영향을 주는 비의도적인 체중감소를 피하는 것이 중요하므로 노인 연령층의 표준체중 기준보다는 적절한 체중을 유지하는 것에 중점을 두도록 한다[10]. 노인당뇨병 환자에 있어 체중감량과 열량제한식사는 권고되지 않으며, 신중하게 고려하고 주의 깊게 적용되어야 한다.

2) 다량영양소

노화에 따른 노인당뇨병 환자를 위한 탄수화물, 단백질, 지방 필요량에 대한 특별한 조정사항은 없으며, 모든 당뇨병 환자를 충족하는 다량영양소의 이상적인 배합 비율은 제시할 근거가 부족하기 때문에 탄수화물, 단백질, 지방의 섭취 비율은 환자의 의학적 상태와 현재 식습관, 개인 선호를 기초로 대사 목표에 따라 개별화할 것을 권고하고 있다[11]. 다만 지나친 저탄수화물식사는 영양불량 우려가 높은 고령에서는 권장되지 않으며[9], 충분한 단백질 섭취가 권고된다[5].

3) 미량영양소

여러 선행 연구들의 근거 부족으로 당뇨병 환자에게서 혈당 개선을 목적으로 미량영양소 보충제를 투여하는 것은 일반적으로 권장되지 않는다. 그러나 저열량 섭취로 인한 미량영양소 결핍의 위험도가 높고 실내 생활만 하는 노인에서는 비타민D 부족의 위험도가 높고, 혈당강하제인 메트포민의 장기 사용과 비타민B12 결핍의 연관성이 보고되어 고령에서 이들 미량영양소 결핍이 확인되거나 이의 가능성이 높은 경우에는 보충제 사용을 고려할 수 있다[9].

4) 노인의 영양관리와 연관된 생리학적 변화

노인 환자에게 동반된 치매나 연하장애, 우울증, 경제적 문제, 암이나 다른 급성 또는 만성질환으로 인한 동반질환으로 섭취 영역, 임상 영역, 행동-환경 영역에 걸쳐 다양한 영양문제가 나타날 수 있다. 노화로 인한 여러 가지 생리학적 변화에 따른 영양문제점을 파악하여 개별화된 영양상담과 식사조절방법 제시, 해당 분야의 전문가에게 의뢰하는 다학제적 접근이 필요하다[10].

결론

노인당뇨병에서 중요한 임상영양치료는 저혈당과 고혈당 발생을 최소화하면서 영양공급을 충족시키고, 개별화된 혈당조절 목표에 도달하는 것이다. 혈당조절의 목표는 연령 자체보다는 신체·인지 기능 상태, 삶의 질과 기대여명을 고려하여 개별화해야 한다. 일반적인 성인에 비해 노인당뇨병 환자에게서 한 가지 주요한 차이점은 체중감량이 반드시 권장되는 것은 아니라는 점이다. 현재 65세 이상 노인당뇨병 관리를 위한 치료적인 목표와 영양 목표의 권고사항이 부족하다. 따라서 노인당뇨병 환자를 위한 실무기반의 임상영양치료 중재는 혈당 및 다른 의학적 치료 목표가 함께 수립되고 논의되어 개발되어야 할 것이다.
